Yes, you can use your FSA or HSA for both trichology treatments and a cranial prosthesis, but they must meet specific IRS medical requirements and have the correct documentation provided the care targets a specific medical condition, and you have a Letter of Medical Necessity (LMN)
The exact same IRS rules govern both accounts. They strictly forbid the use of pre-tax funds for purely cosmetic purposes, including normal, age-related hair thinning or standard pattern baldness. However, if your hair loss or scalp issues stem from an underlying medical diagnosis, the expenses qualify for reimbursement from either account.
1. Cranial Prosthesis (Medical Wig)
- The Rule: The IRS explicitly recognizes a cranial prosthesis as an eligible medical expense, but only if it is prescribed by a physician to treat hair loss caused by a medical condition or treatment (such as alopecia or chemotherapy). Wigs worn for normal age-related thinning or aesthetic reasons do not qualify.
- Documentation: The client needs a prescription or Letter of Medical Necessity (LMN) from their doctor.
- Receipt Tip: The invoice must use the clinical term “Cranial Prosthesis” or “Medical Hair Prosthesis.” If the receipt says “wig” or “hairpiece,” the FSA/HSA administrator will likely reject it.
2. Trichology Treatment & Hair Growth Products
- The Rule: Clinical scalp therapies and medical hair-growth products qualify for FSA/HSA reimbursement when treating an underlying medical condition, autoimmune disease, or physical scalp injury. Eligible conditions include all forms of Medical Alopecia (such as Alopecia Areata, Totalis, Universalis, Ophiasis, Lichen Planopilaris, Frontal Fibrosing Alopecia, CCCA, Folliculitis Decalvans, Pseudopelade, Telogen Effluvium, Anagen Effluvium, and Traction Alopecia). Purely cosmetic, age-related male or female pattern hair thinning is strictly excluded by the IRS.
- Documentation: Because trichologists are hair and scalp specialists rather than medical doctors, the client must secure a signed Letter of Medical Necessity (LMN) from a licensed physician or dermatologist. This letter establishes the care as a legitimate medical treatment rather than an aesthetic cosmetic choice
How FSA and HSA Rules Apply to Trichology
To successfully use your FSA funds for trichology, you must satisfy three critical requirements:
- Medical Diagnosis: The treatment must address a diagnosed medical issue, such as alopecia areata, scalp psoriasis, seborrheic dermatitis, trichotillomania, or hair loss from chemotherapy.
- Letter of Medical Necessity (LMN): For both accounts, you must obtain a signed Letter of Medical Necessity (LMN) from a licensed physician or dermatologist. This letter must state your medical diagnosis (such as alopecia areata, scalp psoriasis, or chemotherapy-induced hair loss) and explicitly prescribe the trichology treatments.
- Provider Status: Because trichologists are hair and scalp specialists rather than medical doctors (MDs), the IRS may scrutinize the claim. Getting your primary doctor or dermatologist to formally refer you or prescribe the specific treatments will heavily safeguard your approval.

- Clinical scalp therapies for severe dermatitis
- Prescription or medicated hair-regrowth topicals
- Low-level laser therapy (LLLT) devices
- Specialized trichological hair-loss evaluations

- Standard hair salon or spa scalp massages
- General non-medicated hair supplements

- A Clear Timeline: How long the treatment is expected to last.
